@mojave - usually the lens used is extremely wide angle, requiring only a few shots to be taken to capture everything in every direction.
It's possible to use a fisheye lens, which can see both straight up and down at the same time, and you only need to take 3 or 4 shots around. If you do it handheld, you can get the spot below the camera while it's pointing straight ahead.
If you're using a tripod though, you'll have a tripod visible in the shot. Cases like that require an extra shot pointing straight down - basically you shoot your panorama, and then you shift over a meter or so, and point the camera down (maybe just hold it with your hands) and then you'll stitch this "down" shot right into the rest of the panorama.

My brother does amazing panoramas with a tripod which you never see in the final product. He is not using a wide angle lens. A panoramic tripod is marked off in degrees. You shoot your photos and then rotate the camera for each shot in a radius, then stitch them together digitally. He usually manages to get them much more seamless than this. Kudos to the photographer for getting good 12 and 6 o'clock positions, however, which are harder to align.
