The shirt is humorous. The creepy Swedish old man smiling while wearing it, not so much. He has the glint in his eye that is awkward. I wonder if this is one of those CafePress generic t-shirt models and he never knew that such fare would adorn him?

Yes, generic T-shirt model.
He was told that people would mostly use his image for variations on "World's Greatest Grandpa", and boy were they wrong.
