The bottom mouth has no teeth missing, but the top one has one missing.
The bottom mouth has something on the right bottom of it and the top one doesn't.
The bottom mouth is slanted, and the top mouth isn't.
The nostril holes are bigger on the top nose.
The top eyes are off to one side, while the bottom ones are looking straight forward.
The top eyes have wrinkles around them and the bottom ones don't.
The top eyes are bigger than the bottom eyes.
The bottom eyes are more slanted than the top eyes.
Not even a close fake. The news media might be able to use this Photoshop user. They don't seem to be too particular about fake pictures, but you're not ready for prime time yet.

you have jumped in assuming its meant to be 2 posters of the same kid, when in fact its obviously not.
Poster not funny until #4 brings us detailed insight into a majestic new world of whimsically satirical win.

I will go to hell by saying this, but the first thing I thought of when I saw this was those "Save the Cleft Palate kids" ads that have been all over lately.
Not that I want to trivialize their condition, it's just that those poor sad little faces are BURNED into my retinas.
